For your convenience, we accept American Express, Master Card, and Visa in addition to cash and personal checks.
Scott M. Sirlin, MD, LLC is not under contract with any health
insurance company. We do provide out of network services to all
PPO's. Patients whose insurance is not an HMO or BlueCross BlueShield,
including Anthem, are asked to pay just their usual co-pay at the time
of each office visit. We will file a claim with your insurance
company. You will be responsible for fees not paid by your insurance
company. HMO and Blue Cross patients, including Anthem, are asked to
pay in full at the time of office visits. We will file a claim with the
insurance company on your behalf. BlueCross BlueShield (and Anthem)
patients who have out of network benefits will be reimbursed directly
by the insurance company, usually within 2 to 3 weeks, whatever portion
of the bill was supposed to have been covered by insurance. Some HMO
patients have out of network benefits. We are not participating
with Tricare or Medicaid.
