To the first office visit, please bring your driver's license, current insurance card, and relevant medical records from previous evaluations, including growth records, lab reports, radiology reports, and biopsy reports.  For infants and toddlers, please bring the most recent dirty diaper so that stool can be checked for blood that is not visible.

 

All patients under 18 years of age must be accompanied by a parent or legal guardian for each visit.
